Informed Consent​
I hereby request and consent to the services of Liberty Saddlery, LLC, that may consist of Saddle Fitting or any other services performed by Lauren Sweeney, who now or in the future, renders services to my horse.

General Release of Liability​

In consideration of the following, I hereby release and forever discharge Liberty Saddlery, LLC from all responsibility or liability of any kind, nature or character whatsoever from the beginning of this world to this day. This transaction is consummated at my specific request.
I understand that any services offered do not replace regular care by my veterinarian, and that saddle fitting practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given would be construed as such.
I agree to keep the saddle fitting practitioner updated as to any changes in my horses' medical profile and understand that there shall be no liability on the saddle fitting practitioner’s part if I forget to inform of such. Should I choose to disregard this liability and pursue legal action, I will pay all legal fees incurred by Liberty Saddlery, LLC.

Appointment Reminder and Health Care Information Authorization​
Liberty Saddlery, LLC may need to use your name, address, and phone number to contact you with appointment reminders. If this contact is made by phone and you don't answer, a message will be left on your voicemail. By signing this form, you are giving us authorization to contact you with these reminders and information.

Office Terms & Conditions During the Appointment​
​
"Office" is defined herein as the physical location in which Liberty Saddlery, LLC is performing services. Please adhere to the following during the appointment:
Safe and clean environment:

NO children running around the horse or in the same aisle as the horse being treated.

Avoid scheduling appointments during feeding time at your location.

Horses must be clean before appointment time.
All services must be paid for at the time of service. Forms of payment accepted for in-person services, during the appointment, are as follows:

Cash, PayPal, and Venmo are accepted.

In state personal checks are accepted but please be aware that a $50 fee will be charged if checks bounce

Responsible adults must be present at all times.

No minor will be the responsible party for a horse at any time.

A responsible adult must be present if the main caretaker of the horse is a minor.
First-time appointment?
The horse owner must be on-site for all services for first time appointments.
Respect for time:

Please be courteous to others and cancel your appointment at least 24 hours ahead of time.

Fittings are charged for up to 90 minutes, if we go over that time frame $25 will be added for each half hour additional.
Saddle Fitting Services Clause

I understand that a horse may need the saddle refitted 2-3 times per year and that this saddle fitting does not replace any evaluation by my veterinarian for any condition. I understand that any measurements taken (if applicable) at this saddle fitting appointment will change with activity of the horse. Any measurements taken (if applicable) for the purpose of custom saddle-making does not make Liberty Saddlery, LLC responsible for the fit of the saddle once it has been made by the saddle maker. Liberty Saddlery, LLC typically deals with saddles already in existence -- not saddles that are going to be custom-made by a saddle maker. If applicable, any further issues with a custom-made saddle must be discussed with the saddle maker from this saddle fitting appointment onwards. None of these services are a substitute for proper veterinary care. Saddle fitters cannot take responsibility for the primary health care of client’s horses.
Saddle Trial Policy

Liberty Saddlery offers fully paid 5-day trials on all saddles to prospective customers within the USA. The purpose and ultimate goal of our 5-day trial period is to ensure that our customers (and their horses) are happy with each saddle purchase. All international orders are final sale only.

Simply order the saddle that you are interested in trying through our website. Once the order and payment have been received, it will be shipped within 2 business days.

The 5-day trial begins the day the saddle is delivered to your doorstep.

If you decide to return the saddle, you must notify us (by email or text) that the saddle is to be returned and we will email you a pdf of the shipping label to print.

If you don't notify us within the 5-day trial period of your desire to return the saddle, the saddle becomes your property.

Once we get the saddle back and confirm that it is in the same condition in which it left, we will contact you within 1 business day to arrange your refund.

Please see our Saddle Trial Agreement for more details around any excessive wear & tear, damage, excessive dirtiness, etc. that should happen to the saddle while it is out on trial.

In the event you return the saddle at the end of the 5-day trial, there is no restocking fee at all if you use PayPal Friends or Venmo. However, if you pay using PayPal Goods & Services, Venmo Purchase Protection or standard credit cards through the site, there will be a minimal restocking fee of 3.5%.

Shipping Policy

Until further notice, you must use any mail carrier other than USPS.

We highly recommend you ship your saddle in a HEAVY DUTY box.

If a saddle is not photo shoot ready upon arrival to our business address, a $25 cleaning fee will be issued.
